Application Information

The first step in submitting an artwork for inclusion in Operation Art is to familiarise yourself with the information contained in the 2009 brochure.  Please use the link below to download or view a copy of the brochure

To complete your entry in Operation Art, the following items must be submitted with your art work.

 

A Student Entry Form must be completed and submitted for each student's entry. This form is on the inside back cover of the 2009 Operation Art Information Brochure sent to all schools at the beginning of the school year. Further copies of the brochure are available from The Arts Unit. Please contact Fiona Steel, Operation Art Coordinator, on 02 9550 9083. A General Release Form must be completed by the parent/guardian of each student artist, and submitted with each entry. This form is on the back cover of the 2009 Operation Art Information Brochure, or can be downloaded from the link below.

A recent photograph of the student artist (passport size preferred) should be attached to the accompany each entry. Please label the back of your photograph with the student's name in case your photo becomes separated from its form.

 

Student artists should also give a brief description of their work in the box provided on the Student Entry Form. This should be no more than 25 words in length and describe the art work, process or intention of the artist.

 

The name of the artist, their class or school year and school name should be clearly printed on the reverse side of the completed work.

 

A cheque made payable the the NSW Department of Education and Training should also accompany your entry.
